Information you submit

The forms collect your name, email, telephone number, service interest, preferred language, and message. Callback requests may also include a preferred format, date, and time.

How information is used

Information is used to review and respond to your request. Submitted requests are stored in the website’s protected request inbox, which only authorized Smart Tax staff can access, and are sent to the Smart Tax office email address. Submitting a form does not confirm an appointment or create a tax service engagement.

Appointments through Zoom

Booking buttons take you to Zoom Scheduler. Information entered there is processed under Zoom’s terms and privacy practices.

Sensitive documents

These forms are not designed to receive tax documents. Do not submit Social Security numbers, bank information, or tax records. Contact the office to agree on an appropriate document-sharing method.
